As of January 21st, the two pages listed below represent all the info that is currently available for our second trip to Salisbury as a chorus. More will be available in the next few months.
[ These two pages are extracted from the 'Adobe Acrobat (pdf) registration' form, uploaded from Mid-Atlantic District's site, and modified to meet our needs. I have not yet received approval to use the modified registration form.]
I. This first page, 2004 Mid-Atlantic District HARMONY COLLEGE EAST 2004, is not yet approved, but is the registration form that will be printed for every participant to fill in. Read the instructions above each blue-bordered frame.
The top frame should be filled in with Name 1, Address, City, State, Zip, and phone # (Quartet registrants use one form for all four names, and additional info in the Quartet blocks. Also notice the additional requirements:biography,etc! ). The area labeled Registration Options/Fees: should have only one check-mark in it, and only the first 2 lines apply (except for quartets). Those who are not taking additional classes should opt for the appropriate 2n(ight)+6m(eal) rate [sorry, the 2night+3meal option is NOT available - administratively impossible].
The second frame is for selecting Thursday and Friday classes for those who choose to do so. There should be only one '1' and one '2' in each column, and of course: if there is a '1' in column 4, there CANNOT be a '1' in column 2 or 3 (unless you have figured out how to be in two places at once!).
II. The second page, 2004 M-AD HCEast info 2004, is class descriptions and some additional information for planning purposes. Some Saturday-only classes were removed to fit on one printed page.
